The Opportunity
We are seeking a motivated Project Manager – Performance Specialist (or Jr. Project Manager, depending on experience) to join our Performance Improvement Team. In this role, you will lead and support population health initiatives, practice performance programs, and provider engagement efforts that directly improve patient outcomes and organizational success. This is a dynamic opportunity to combine project management, data analytics, and provider collaboration in a role that impacts both clinical quality and financial performance.
What You’ll Do
Lead and manage multiple population health and performance improvement initiatives (e.g., Annual Wellness Visits, Transitional Care Management).
Analyze practice data to identify trends, risks, and opportunities for improved outcomes and revenue growth.
Support the rollout and monitoring of the Performance Enhancement Series (PES) program for underperforming practices.
Create dashboards, reports, and presentations to track progress and communicate results to leadership and providers.
Coordinate provider and consultant communications, including performance feedback and improvement plans.
Manage scheduling and documentation for Physician Advisor (PA) program meetings.
Conduct quarterly performance reviews and action plans for participating practices.
Educate providers and staff on population health initiatives, workflows, and quality expectations.
Collaborate with cross-functional teams to improve processes, resolve issues, and ensure HIPAA compliance.
What We’re Looking For
Required : Bachelor’s degree in Business Administration, Healthcare Administration, or related field.
Preferred : Master’s degree plus 3+ years of experience in healthcare project management, hospital operations, or clinical performance improvement.
Strong organizational skills with high attention to detail and confidentiality.
Knowledge of healthcare performance metrics, population health strategy, and value-based care.
Proficiency in Microsoft Office (Excel, Word, PowerPoint); SQL or reporting tools a plus.
Excellent communication and relationship-building skills with physicians, staff, and leadership.
