A company is looking for a Community Support Coordinator to bridge the gap between clinical and non-clinical support services for members in population health and respiratory care programs.
Key Responsibilities
Serve as the primary non-clinical contact for members, conducting outreach calls and assessments
Coordinate patient transportation and assist in scheduling follow-up appointments with healthcare providers
Document all member interactions and maintain confidentiality in accordance with HIPAA regulations
Required Qualifications
Three (3+) years of administrative or office management experience, preferably in healthcare
High school diploma or GED required; Associate's degree preferred
Familiarity with healthcare operations and population health programs is desirable
Proficiency in Microsoft Office Suite and electronic health records (EHR)
Knowledge of HIPAA-compliant data handling practices
