A company is looking for a Case Manager - Behavioral Health - Full Time.
Key Responsibilities :
Coordinate and provide safe, effective, and client-focused care
Monitor and evaluate client progress through case management plans
Educate and support patients transitioning to community-based services
Required Qualifications, Training, and Education :
Master's degree in Social Work, Mental Health Counseling, or related field
Experience in a social service setting with knowledge of case management systems
Ability to assess crisis situations and respond appropriately
Familiarity with documentation and compliance with guidelines
Strong understanding of mental health and substance abuse disorders
