Salary : DOE : RN - $41.20-$61.80 / LPN - $28.15-$42.25

Nurse Visit RN / LPN

Location : Northwest Human Services West Salem Clinic, 1233 Edgewater St. NW, Salem OR 97304

Job Status : Full-time, Monday - Friday, 8am 5pm with rotating Saturdays

Sign-on Bonus : $3,000.00 (Internal candidates that are currently employed here at NWHS are not eligible for sign-on bonus.)

Referral Bonus : $1,000.00

NWHS is currently seeking a compassionate and proactive Nurse Visit RN / LPN to join our Patient-Centered Medical Home (PCMH) team, where you'll support high-quality, nursing care visits that promote prevention, chronic condition management, and care gap closure.

YOUR ROLE :

As our Nurse Visit RN / LPN, youll work side-by-side with providers, behavioral health professionals, medical assistants, and care coordinators to empower patients through education, access, and coordinated services that improve health outcomes.

SPECIFIC DUTIES :

• Participate in nurse visits for services such as immunizations, wound care, diabetes support, and contraceptive injections under the clinical direction of an RN or licensed provider.
• Collaborates with care team to ensure seamless and patient-centered care.
• Reinforce patient education related to prescribed medications, chronic condition management, and proper use of medical devices, as directed by the care plan.
• Perform focused assessments and collect clinical data (e.g., vital signs, symptoms, patient history) to support the RN or providers evaluation.)
• Ability to recognizes abnormal and critical clinical findings and following established protocols and take appropriate action including notifying provider as appropriate.
