A supportive housing program in New York City is seeking a Service Coordinator to join their team. In this role, the Service Coordinator will provide housing support and case management to participants living in scattered-site housing throughout the community.

- Schedule : Monday to Friday
- Hours : 9am to 5pm
Responsibilities :
Act as a liaison between participants, building superintendents, and property managers to ensure timely apartment repairsMaintain detailed logs of apartment and building conditionsReport non-compliance or urgent housing issues to the Supportive Housing DirectorProvide case management and on-site crisis interventionComplete service plans and document monthly progress in participant chartsConduct home visits to assess compliance and identify service needsFacilitate monthly tenant meetings and educate on home safety, budgeting, and landlord relationsEscort participants to social service agencies as neededReport suspected child abuse or neglectComplete annual rent calculation and apartment inspection formsUpdate monthly program data and ensure lease documents are current and filedQualifications :
3+ years of experience in a Behavioral and Mental Health settingAssociate's Degree or equivalent in a related fieldStrong organizational, communication, and interpersonal skillsAbility to use sound judgment and maintain discretion with sensitive issuesCulturally competent and committed to diversityComfortable multitasking in a fast-paced environmentDesired Skills :
BilingualExperience in Supportive Housing and / or Social ServicesFamiliarity with Housing programs and Compliance procedures
