We are looking for passionate, tenacious Canvassers who can build strong, authentic relationships with our members. In this role, you will be instrumental in helping community members access Enhanced Care Manager (ECM) and CalAIM services, while also facilitating Wider Circle's Connect for Life program. You will engage directly with individuals, making in-person visits to members' homes, connecting them to local resources, and motivating them to participate in health-focused community activities.

Wider Circle works with health plans and providers nationally to deliver unique community care programs that connect neighbors for better health. Centered on trusted relationships, Wider Circle connects health plan members with like-minded neighbors to inform, support and motivate one another, empowering them to be more proactive about their health. Wider Circle's trusted delivery network has been proven to drive resilience, improve member experience and engagement, and reduce inappropriate utilization and has been published in peer-reviewed literature. Today, Wider Circle offers its unique neighborhood care programs to tens of thousands of communities nationwide.

Responsibilities :

• Meet members at their homes to introduce and explain our programs.
• Discuss and promote the benefits of ECM and CalAIM services.
• Facilitate in-person Connect for Life activities focusing on trust-building, health promotion, and movement.
• Build and nurture relationships with members through regular follow-up, both in person and over the phone.
• Encourage members to participate in community meetings and recruit new members to join.
• Serve as a liaison between members and their health plans, ensuring they understand their benefits and options.
• Partner with local community organizations to offer additional services and resources to our members.
