Community Support Specialist – Sertoma Star Services
Overview
As the Community Support Specialist, you will work as part of the Community Support Team (CST) to provide support services to individuals assigned to CST in order to decrease crisis episodes and increase community functioning. You will assist consumers to achieve recovery goals, conduct assessments, develop treatment plans in collaboration with the consumer and other stakeholders, and complete required documentation in accordance with applicable standards and Sertoma Star policy. On-call support is required on a rotating schedule. At least 60% of CST services are delivered off-site in community settings using the vehicle resources provided by the organization.
Responsibilities
- Provide direct CST services, with at least 60% off-site, in the consumer's natural environment in accordance with the service plan.
- Complete all required assessments (including IATP, LOCUS) and documentation, including billing notes, per Rule 132 / 140 standards and payer / accreditation requirements.
- Develop and implement crisis plans and treatment plans in collaboration with the consumer and CST, following applicable standards and requirements.
- Collaborate with CMHC staff to ensure coordinated care and provide referrals when CST eligibility ends.
- Provide services aimed at reducing crisis episodes and increasing community functioning toward rehabilitative and recovery goals.
- Maintain CST records per Sertoma policy and participate in quality assurance activities.
- Attend weekly team meetings and maintain up-to-date knowledge of each consumer's treatment plan and needs.
- Maintain open communication with agency staff, families, and other stakeholders; facilitate access to services as identified in the treatment planning process.
Qualifications
Bachelor's Degree in a human services field or equivalent Mental Health Professional experience (bachelor's degree plus 5 years of supervised mental health / human services experience).Minimum of one year experience working with individuals with mental health and / or substance use disorders.Effective oral and written communication skills.Valid driver's license with current auto insurance and acceptable motor vehicle report.Proficiency with computers and Microsoft Office Suite.Base pay and Benefits
Base pay range : $45,000.00 / yr - $50,000.00 / yr
