Social Services and Activities- Healthcare

POSITION SUMMARY :

The Social Worker is responsible for providing direct services to patients and their families. This includes counseling and seeking resources to answer unmet needs, as well as providing consultation services and in-service education programs related to the psychosocial aspects of health care to patients and their families.

QUALIFICATIONS :

• Bachelors Degree in social work or related field, Masters Degree preferred
• Licensed in the state as a RSW, GSW, MSW preferred
• Three to five years experience working with SNF residents and their families
• Basic knowledge of computer skills – Microsoft Office (Word, Excel, Powerpoint)
• Must be able to read, write, and speak English

PHYSICAL AND MENTAL REQUIREMENTS :

Mental : Stress Level : Moderate to High

EXPOSURE RISK :

Category II – Job in which required tasks normally do not involve exposure to blood, body fluids or tissues; but may require performing unplanned Category I tasks.

Computer, Copier, Telephone, Fax Machine, Calculator, Printer.
