Temporary assignment (Sept–Dec 2025, with strong potential to extend or convert to permanent)

$39.84 – $56.91 / hr (based on location; additional $3–$5 / hr possible for highly qualified candidates)

About the Opportunity

We’re seeking a Licensed Clinical Social Worker (LCSW) to support a new Dual Eligible Special Needs Plan (D-SNP) Care Coordination program. This temporary role runs through the end of 2025, but there’s a high likelihood of extension into 2026 or conversion to a permanent position.

This is a hybrid / remote opportunity. While preference is given to candidates located in Monterey, San Benito, Santa Cruz, Mariposa, or Merced Counties, we’re open to fully remote applicants anywhere in California.

What You’ll Do

• Develop and manage individualized intervention plans for Medicare D-SNP members with complex needs
• Collaborate with an interdisciplinary care team to support members, caregivers, providers, and community partners
• Help members navigate the managed care system and connect with needed resources
• Conduct culturally competent assessments, interviews, and advocacy on behalf of members
• Contribute to program evaluation and quality improvement to ensure effectiveness and compliance

What You’ll Bring

Bonus skills : Medicare / Medicaid experience, prior health plan or payer experience
