Opportunity :

Advanced Practice Provider - Elder Care

Job Details :

• Full-time (1.0 FTE) NP / PA position providing gerontology-focused care to patients
• Utilize advanced health assessment, knowledge, and decision-making skills both independently and in collaboration with physicians
• Provide health care to individuals and families with emphasis on health promotion and disease prevention
• Work onsite at patient residences, reducing burden on families and patients who would otherwise need to travel to clinics
• Must be comfortable working autonomously offsite with collaborating physician partner available by phone
• Schedule : 7 : 00 a.m. to approximately 5 : 00 p.m. daily
• Involves after-hours on-call rotation, including weekends and holidays
• Requires high level of customer service skills to establish positive relationships with patients, co-workers, and others
• Education required : Master's or Doctorate degree in Nursing or Master's degree in Physician Assistant Studies from an accredited program
• Must have current licensure as a Nurse Practitioner or Physician Assistant in Minnesota
• Wisconsin licensure required within 3 months of hire
• National certification in applicable area and current CPR certification required
• Successful completion of credentialing process prior to practice
• Ideal candidate will have experience as an RN or NP / PA with elder care background

Compensation :

Competitive hourly rates with weekly pay and malpractice included. May be eligible for additional compensation in the form of bonuses, quality incentives or production-based compensation.

Estimated Annual Salary : $ 120000 - $ 140000
