Position Overview
The Clinical Nurse is a Registered Professional Nurse who provides safe, competent, quality care based on nursing theory and research to a designated group of patients and their significant others in an ambulatory care setting. This role emphasizes Relationship Centered Care and the nursing process through evidence-based practice and approved organizational nursing standards.
Duties & Responsibilities
Patient Care
Deliver patient care by applying the nursing process, research, and evidence-based practice.Assess and evaluate patient care needs, applying critical thinking in care management.Collaborate with interdisciplinary healthcare teams to develop, implement, and evaluate care plans.Prioritize patient care activities including teaching, rounding, coaching, and after-hospital planning.Educate patients and caregivers regarding care plans, transitions, health promotion, and disease prevention.Communicate effectively and professionally with patients, families, and healthcare team members.Manage assignments within the Care Delivery Model of Modified Primary Nursing.Patient Experience
Role model Mount Sinai Relationship Centered Care through various patient-centered actions such as Admission Welcome, Bedside Shift Report, Purposeful Hourly Rounding, and Discharge Wrap-Up.Consider patient values, preferences, cultural diversity, and expressed needs in care.Establish and maintain therapeutic relationships with patients and families.Utilize evidence-based practices to improve understanding of patient perceptions of care.Address patient and family concerns through service recovery and escalate when necessary.Demonstrate caring, respect, empathy, and active listening in all interactions.Quality and Safety
Practice safe and efficient patient-centered care, identifying and escalating barriers.Demonstrate clinical skills essential to quality and safety.Participate in quality improvement projects, evidence-based practice activities, and nursing research.Implement principles of high reliability to avoid high-risk behaviors and maintain a safe environment.Utilize institutional information technology to document nursing practice supporting quality initiatives.Operations
Perform charge nurse duties as assigned.Coordinate with management to ensure proper use of supplies and resources.Document accurately and thoroughly in compliance with legal and regulatory requirements.Facilitate patient throughput on a shift basis.Professional Development
