Permanency Case Manager

Our Permanency Case Manager works to meet the goal of permanency for children in our program, in a caring, responsible way, following all standards and policies.  You will work with children and families during that time to match them with the best placement option, educate families on the adoptive process, implement a permanency plan, coordinate services and assistance to improve the social and psychological functioning of the participants, conduct home studies and assessments, coordinate subsidy, and assist with submitting documentation for adoption and guardianship.  This position serves as an advocate and partner with families to assist them through the permanency process, including attendance at court hearings, school, and community meetings.  You will attend events to network, educate on the need for foster / adoptive parents, and develop relationships to attain potential foster parents.

What we are looking for :

Bachelor’s degree in an approved Human Service field

CWEL

Valid Driver’s license in good standing, and current automobile insurance.  Must obtain a valid IL license within 30 days of hire if moving from out of state.

CPR and First Aid Certified, or ability to be certified within 30 days of hire.
