The Intake Case Aide is responsible for receiving, documenting, and managing referrals made to our Case Management Agency for folks seeking Long Term Care Services and Supports. The Intake Case Aide will utilize several different modalities including phone, email, and fax to receive and process referrals with timeliness and attention to detail. The Intake Case Aide will answer questions about how to access services and supports and provide information about available state programs, waivers, and providers related to an individual's specific support needs.

Primary Responsibilities :

Utilizing a professional and courteous demeanor, provide excellent customer service for customers and various internal departments and individuals in accordance with the mission, and values of the company

Interact with internal and external departments, agencies, advocates, medical providers, families, and individuals to connect members with what they need

Frequently handles conversations with individuals of varying emotional states. Responsible for diffusing the situation, providing information and empathy, and ensuring clients and their families are cared for as much as possible

Provide first point of contact coverage virtually in a cordial and professional manner. Determine their needs and provide information and / or notify the appropriate individual

Receive and route incoming calls as needed to ensure effective customer phone service. Respond to internal and external contacts in a patient, friendly and helpful manner

This position does not require travel

Include attendance at workgroups or regulatory meetings

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $17.74 to $31.63 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
