Attractive Compensation Package $78,000-$85,000 plus quality incentive quarterly payout for excellence in clinical quality and after hours pay.

A Day Life as a Hospice Nurse Role of the RN Case Manager As a Hospice Case Manager, you will have a special opportunity to provide, coordinate and direct the provision of hospice nursing and palliative care to terminally ill patients in their home and wherever they call home. Our Hospice nurses will closely collaborate with the attending physician, patients and their families, and other members of the patient care team to provide and maintain continuity of patient care to achieve excellent pain and symptom management and high-quality end-of-life care for the patient. We service an adult population providing support to patients and families. Nuts & Bolts of the Job Hospice RN Case Managers work Monday - Friday with a potential on call and Holiday coverage schedule. Case Manage 15-18 patients, attend Interdisciplinary Team Meetings, contribute to team huddle calls, and receive updates on your patients daily. Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so. Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies. Records pain / symptom management changes / outcomes as appropriate. Teaches patients, families, and caregivers appropriate patient care techniques. Regularly communicates with clinical manager and care team. Performs on-call responsibilities and on-call services to patients / families as assigned. Other duties as assigned. Qualifications 1+ year(s) of nursing experience in acute care setting(s), ER or fast-paced environment(s). Current RN license, specific to the state(s) you are assigned to work. Current CPR certification. Valid driver's license, reliable transportation and liability insurance. HCHB experience and / or acute care nursing roles a HUGE plus!
