This position will require Case Management nurse experience. Rehab experience preferred.

Job Summary And Qualifications

• You will ensure high quality, patient-centered care by managing rehabilitative care to ensure optimum outcomes
• As an active member of the rehab team, you will provide, coordinate, and direct care specific to the needs of each Rehab patient
• You will collaborate with the Rehab Program Director and Facility Case Management Director regarding departmental functions, and coordinates efforts within the rehabilitation team
• You will provide thorough verbal and / or printed information to help patients / families / caregivers make informed decisions about post-acute care options, while addressing goals of care and treatment preferences
• You will facilitate discharge and arrangement of follow-up services, implementation of discharge / transition recommendations, and identify and utilize appropriate community resources to meet patient discharge and continuity of care needs
