The Registered Dietitian responds to all Nutrition referrals generated by NeighborHealth providers. The Registered Dietitian provides clinical assessments, develops care plans, educates patients and families, and provides follow-up.
The Registered Dietitian also participates in Health Center functions to promote health and wellness and contributes to the management of chronic health conditions in accordance with evidence-based guidelines for medical nutrition therapy. The Registered Dietitian provides individual as well as group counseling. Registered Dietitians participate in NeighborHealth's Nutrition Department activities that support clinical excellence.
DUTIES :
- The RD conducts clinical assessment, develops care plans, and coordinates interventions to support participants' nutritional health and management of chronic health conditions consistent with PACE policy and procedure and evidence-based guidelines.
- Completes nutrition assessments and implements care plans at enrollment, annually, and at status changes appropriately and in a timely manner.
- RDs oversee nutrition-related competencies for Dietary Assistants in the PACE Center and all PACE Health Aides.
- Demonstrates the ability to work collaboratively with the participant / caregiver and other members of the IDT in the development and implementation of team goals and related plan of care.
- Refers participants to appropriate in-house and community services, and to other providers, as appropriate (including speech therapy, meal services, SNAP, Wellness Center)
- Oversees Nutrition Services, including PACE Center-based meals, home-delivered meals, as well as grocery delivery service.
- Maintains State and National registration / licensure.
EDUCATION :
Master’s Degree required.Registered Dietitian (RD)Maintains State and National registration / licensure.Maintains the Academy of Nutrition & Dietetics Professional Portfolio.EXPERIENCE :
Minimum 1 year as a Registered Dietitian with clinical and public health experience.SKILLS / ABILITIES :
Ability to work in a team environment.Significant interest and the ability to work within the context of the Chronic Disease Management Model, including patient self-management goal settingVersed in or willing to quickly develop skills in motivational interviewing and concepts of behavior changeEvidence of creativity, flexibility, and professionalism.Ability to work successfully in a peer-to-peer environment.Bilingual in English / Spanish is a plus.
