Job Title : Home Health Case Manager – Registered Nurse (RN)

Location : Sacramento, California

Shift : Day Shift | 5x8-Hour (08 : 00–16 : 30)

Employment Type : Full-Time

We are seeking an experienced and dedicated Home Health Case Manager RN to join our team. This role requires a strong background in home health nursing and case management, with a focus on delivering high-quality, patient-centered care.

Key Responsibilities :

• Manage a designated caseload of home health patients, ensuring timely and effective care delivery
• Provide direct patient care in accordance with visit productivity standards
• Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team
• Coordinate care across disciplines to optimize patient outcomes and resource utilization
• Monitor visit utilization and manage associated expenses
• Participate in agency-wide orientation, competency evaluations, and provide consultation to clinical staff as needed

Required Qualifications :

Active California RN license

BLS certification

Minimum experience in Home Health Case Management
