What Youll Do
The Case Manager develops trusting client connections, providing supportive counseling and evidence-based case management services. Key responsibilities include :
Screening & Assessment Conduct biopsychosocial evaluations; determine severity of SUD / mental health needs.Level of Care Placement Partner with medical / clinical staff to recommend appropriate treatment levels.Client Orientation Guide clients through program expectations, policies, and resources.Case Management Develop treatment plans, coordinate care with peer support and clinical teams, and present in team meetings.Treatment Planning Collaborate with clients on individualized SMART goals tailored to strengths and needs.Care Coordination Connect clients with providers, support specialists, and community resources.Education & Advocacy Educate clients on recovery options and advocate for their needs and those of their families.Documentation Complete timely, accurate clinical records in accordance with regulations and organizational standards.Qualifications
Bachelors degree in human services or related field required; Masters / MSW preferred.2+ years case management and assessment experience strongly preferred.Experience in 12-Step programs and / or dual diagnosis treatment preferred.Current First Aid / CPR certification (or ability to obtain within 30 days of hire).Strong communication, collaboration, and advocacy skills.Our Values
