Description
Job Duties :
1. Provide assessments, individual, group, and family therapy in compliance with DMH standards while focusing on strengths, needs, abilities, and preference of the youth and family.
2. Provide clinical support to individual youth and family to help them cope with stressors related to serious emotional disturbance.
3. Actively collaborates with other systems' staff regarding assigned individuals and their families.
4. Monitor youth / family participation and progress in organized treatment programs to assure the planned provision of service according to the youth's individual treatment plan.
5. Participate as a member of multi-disciplinary team in all treatment plan review of the youth.
6. Conduct home visits for regular monitoring.
7. Provide ongoing education of youth and family regarding clinical treatment.
8. Address concerns that interfere with family functioning and dynamics.
9. Maintain 20 to 30 cases of youth and their families.
10. Collaborate with families and systems to assist in developing the individualized treatment plan.
11. Maintain contact with youth who are hospitalized / respite / residential and participate in discharge planning and necessary collateral contacts.
12. Provide parenting education when appropriate.
13. Seek culturally competent services and service providers to meet the unique cultural needs of youth and family.
14. Complete all mandatory documentation to monitor youth and family progress towards achievement of individualized treatment plan goals.
15. Perform outreach and engage with culturally and linguistically diverse community-based organizations, cultural groups, providers, and agencies.
16. Identify formal or informal leaders in the cultural communities and engage them in decision-making or advisory roles.
17. Have a presence in the community and ask permission to engage in the community's cultural activities and events.
18. Conduct home visits for regular monitoring.
19. Demonstrate appropriate interaction with coworkers that promote a pleasant work environment.
20. Maintains client confidentiality at all times.
21. Completes additional duties assigned by the supervisor.
Key Responsibilities
Requires clinical licensure / DMH certification within 30 days of hire and maintained throughout the duration of employment.
Requires excellent time management, organizational, and interpersonal skills.
Requires satisfactory completion of CPR prior to reporting to the work site.
Requires satisfactory completion of CPI within 30 days of employment.
Requires valid driver's license and current auto liability insurance.
Requires the ability to transport clients in private vehicle when necessary.
Requires completion of annual trainings in compliance with DMH standards.
Previous experience in a mental health setting is strongly preferred.
Previous experience working with SMI / SED or IDD populations in treatment settings strongly preferred.
Ability to think independently is strongly preferred.
Visual and auditory acuity sufficient to evaluate client / staff needs is preferred.
Fine motor skills for legible and accurate writing of reports, scheduling, daily correspondence and presentations is preferred.
Efficient use of time management by keeping thoughts free of obstruction is preferred.
Working knowledge of the DSM V
