Care Manager Supervisor
Job Summary
Supervises assigned Care Managers and Unit Clerk. Responsible for coordinating and managing the discharge plan for assigned patients. The position leads in collaboration with the referring treatment team to assure all program metrics are met. Assures the assigned patients safely engage in treatment at their next level of care. Completes all documentation and reports in an accurate and timely manner.
Essential Functions
Implement the discharge plan for all patients at the time of admission into the service.
Facilitates a smooth hand-off from referring department.
Assures all screening tools are performed and documented in the patients’ medical record.
Engage patient in discharge planning, if patient refuse to participate in discharge planning document refusal in patient’s medical record.
Educate the patients of the importance for follow up care.
Assure transition record is sent to the referral provider and insert proof into the patient’s medical record.
Assist with or provide transportation to ensure engagement with follow up appointments.
Make reasonable effort to engage family members and / or legal guardian in discharge planning, as clinically indicated.
Participate in multi-disciplinary treatment team meeting to effectuate the discharge care plan.
Utilize community resources to meet the discharge needs of individual patient needs.
Ensures continuity of care for patients with complex discharge needs.
Document in patient’s medical record on the day of service.
Provides in-home / in-community outreach to assure patient engagement.
Performs other duties based on departmental needs as assigned by management.

Minimum Education / Certifications
Bachelor’s Degree in human services or related field
Valid New Jersey driver’s license insurable by the hospital
Computer Literacy
Excellent Communication and interpersonal skills
Minimum Work Experience
Two years behavioral health or related experience
One year of case management experience
Knowledge of New Jersey’s mental health system
Position Type / Expected Hours of Work
Day Shift; 8 : 30AM - 5 : 00PM
Must be open to rotating weekend shifts as needed
Physical Demands
Language Skills : Ability to read, analyze, interpret, and evaluate information presented in research reports, professional publications, referral documents and similar materials. Ability to write clearly and cohesively and to present information effectively both orally and in writing.
Reasoning Ability : Ability to define problems, collect data, establish facts, and draw valid conclusions from a wide variety of data and deal with several abstract and concrete variables.
Physical Demands : Ability to climb stairs; facility does not have elevator. While performing the duties of this job, the employee is regularly required to : sit; use hands to finger, handle or feel objects, implements or office equipment; talk and hear; stand and walk, reach with hands and arms; stoop or kneel. Employee may occasionally lift to 20 pounds or assist in lifting patients in emergencies. May work at keyboard intermittently throughout the day. Specific vision abilities required by this job include close, distance and color vision and the ability to adjust focus.
Work Environment : The noise level in the work environment is usually quiet to moderate.
