The Social Services Director plays a key role in delivering medically related social services to attain or maintain the highest level of physical, mental, and psychosocial well-being.

Responsibilities

• Lead social services to support residents' physical, mental, and psychosocial well-being.
• Coordinate with interdisciplinary teams, families, and external agencies to ensure comprehensive resident care.
• Assist with discharge planning, crisis intervention, and case management as part of resident care.
• Foster a compassionate, collaborative, and compliant care environment in alignment with regulatory requirements.
