Who We Serve
Pacific Clinics serves children, transitional-age youth, families, adults, and older adults. We offer a full range of mental and behavioral health services, foster care and social services, housing, continuing adult education and early childhood education programs to Medi-Cal-eligible individuals and families throughout Alameda, Contra Costa, Fresno, Kings, Los Angeles, Madera, Orange, Placer, Riverside, Sacramento, San Bernardino, San Francisco, Santa Clara, Solano, Stanislaus, Stockton, Tulare and Ventura Counties.

About our Program : Placement Supportive Services Level 1
is a 60-day program with a Professional Parent model designed to provide a comprehensive integrated service model designed to meet the needs of children / adolescents who are at risk of out-of-home placement, abuse, and neglect. Services will be tailored to provide support to children and their families who are represented in the Child Welfare System. We serve the highest acuity youths in the county and have a 30-minute response time for our youths, and can provide 24 / 7 crisis stabilization support, if needed / approved by DFCS.
Placement Supportive Services Level 2
Is a 60-day support to stabilize youth who are at risk of losing their current placement, as well as supporting the families who are or have been connected to DFCS. PSS2 youth have a 24 hours response time.

JOB SUMMARY
Works within the vision, mission, and philosophy of the agency overseeing a program that provide 24 / 7 crisis response and stabilization services to individuals experiencing a mental health crisis. Intervenes with crisis behaviors while developing the individuals strengths and interests. Responsible for linking to community resources. Does whatever it takes to meet the needs of the individual in a strength-based approach. Helps to create permanent connections for individual. May search, locate, and engage individuals' natural supports. Assists with transitioning individual to the lowest level of care needed. Works independently in the individuals' or youth's home and in a variety of community and / or treatment settings.
