Job Description

Essential Job Functions and Responsibilities :

• Performing and documenting initial evaluations and follow-up treatment visits in accordance with state practice act and consistent with current best-practices.
• Establishing appropriate plan of care for patients including prognosis, short / long term goals, and complete documentation in a timely manner.
• Prioritize a patient centered approach with a focus on patient's desired goals and function.
• Regularly reassess patient progress and modify plan of care as necessary.
• Communicate effectively with clinic staff, physicians, and other members of the patient's healthcare team
• Providing supervision for Physical Therapist Assistants, ancillary staff, students and volunteers as required
• Maintaining a full patient caseload and remaining flexible to accommodate patients. Follow-up treatment sessions are scheduled for 30 minutes one-on-one with each patient.
