Home Care Team Manager

Position Summary : This position is responsible for ensuring the delivery of quality case management to elders in the community through hiring, training, developing, supervising, and coaching case managers.

Principal Accountabilities :

Ensures that case managers receive supervision and direction :

• Provides supervisory oversight of case managers by maintaining an open door policy and conducting regular supervisory meetings to monitor and evaluate performance.
• Identifies training needs and coordinates training.
• Conducts team meetings; coordinates team activities.
• Oversees and coordinates specialized programs including Enhanced Community Options Program Independence Plus, Veterans Independence Plus, and Consumer Directed Care.

Ensures high quality case management :

Ensures that case managers are trained and developed :

Ensures that service plans conform to the current spending plan for each home care program :

Qualifications :

Education : Bachelor’s degree in human services or related discipline.

Experience : Minimum of two years elder services or other human services experience.

Special Skills / Knowledge : Excellent oral and written communications skills, ability to effectively relate to a wide variety of personalities, basic computer skills, organizational and time management skills, ability to multi-task and work in a fast-paced, high volume work environment, demonstrated leadership ability. Driver’s license and reliable transportation required.
