American Traveler is seeking a Clinical Care Coordinator (RN) in Acute Case Management with recent CA Dignity / CommonSpirit acute care experience and proficiency in utilization review systems.
\n \n Responsibilities\n
Position is in an acute hospital care setting within the Care Coordination (Case Management) departmentAdult patient population with a 1 : 20 patient assignment ratioResponsibilities include case management and utilization reviewUses Midas, InterCall, Cerner EMR, Curaspan, and Microsoft Office8-hour day shifts with every other weekend required and first to floatPotential for 12-hour night shifts if experienced in ED and willing to work nightsFloating required to all adult areas of the hospital, excluding areas without relevant experienceBusiness casual, conservative dress code with lab coats requiredMust be able to adapt quickly, demonstrate flexibility, and work with minimal supervisionResponsible for care coordination, discharge planning, and efficient utilization reviewMonth-long assignment includes four days of web-based charting orientation and two days with a preceptorMust provide coverage for two of the three major holidays (Thanksgiving, Christmas, New Year's); specific holiday time off not guaranteedExcellent attendance record is expected; minimal time off requests preferredCandidates may not have been directly employed by any CommonSpirit, CHI, or Dignity facility within the past yearLab coats must be worn with business casual attire; no open-toed shoes or strong fragrances permitted\n \n Requirements\n
Active California RN license required at time of considerationCurrent BLS certification requiredMinimum of 3 years hospital experience, including acute care coordinationExperience with Midas, InterCall, Cerner, Curaspan, and Microsoft Office is requiredDemonstrated proficiency in discharge planning and utilization reviewMust have recent CA Dignity / CommonSpirit acute care experienceTwo professional references required : one supervisor from the past 12 months and one supervisor or peer from the past 3 yearsCopy of driver's license required for considerationRNs must pass Relias Dysrhythmia exam and sterile technique test during orientation\n Benefits\n
