RN - Coordinator - Transfer Program - Full Time - 12 Hour - Nights

Job Description :

The RN Coordinator - Transfer Program is responsible for the management of transfer patient placement within the acute medical and psychiatric hospitals for patients requiring admission and / or internal and external transfers. The Coordinator acts as a liaison to physicians, departments and the nursing leadership team to assist in the timely placement of patients requiring acute care admissions in accordance with clinical standards and guidelines and regulations. The Coordinator evaluates and facilitates resolution of barriers to discharges in order to facilitate patient throughput of admissions and transfers to the correct level of care. The Coordinator has command of all patient placement / tracking information systems and will utilize them in decision making to ensure optimal patient throughput. The Coordinator acts as role model for staff in professional behavior, appearance and following work rules and practices.

Education :

Graduate of an Accredited School of Nursing - Required

Experience :

3 years Nursing - Medical Acute Care or

Nursing - Behavioral Health - Required

Certifications / Licensures :

RN Registered Nursing - California Board of Nursing - Required

BLS Basic Life Support - American Heart Association - Required

Work Shift :

12.0 - 19 : 00 - 08 : 00 No Waive (United States of America)

Pay Range : $91.00 - $122.63

Hourly

Offer amounts are based on demonstrated / relevant experience and / or licensure.

Pay will be adjusted to the local market if hired outside of the Bay Area .

Note : Positions at JMH which are exempt (not eligible for overtime) under the level of Manager are listed as hourly for compensation purposes on this posting. The work shift will contain the word 'exempt' on it.
