The Opportunity :

Are you ready to make a profound impact in the lives of patients and their families? As a Hospice Nurse at Saint Luke's Hospice House , you will provide compassionate end-of-life care and symptom management for patients transitioning from the ICU. Many of these individuals require specialized care, including drips, LVADs, ventilators / BiPAP, or tracheostomies. Your role will involve educating patients and their loved ones, administering medications, and collaborating with a multidisciplinary team to create a comprehensive care management plan.

Shift : Full-time, 3 night shifts (12 hours each) per week.

The Work :

• Deliver direct nursing care to patients with terminal illnesses.
• Coordinate care for both patients and their families.
• Conduct initial assessments of physical, emotional, and spiritual needs.
• Implement aggressive pain and symptom management, including medication administration and IV infusion adjustments.
• Provide education and support regarding end-of-life issues and imminent death.
• Maintain an open, sensitive, and non-judgmental presence, respecting diverse beliefs and values.
• Utilize electronic documentation to record symptoms, interventions, assessments, and education provided.
