Position Summary
As a PRN LPN at Luna Del Valle Hospice, you will deliver direct patient care in the home setting, under the supervision of a Registered Nurse (RN). This role is essential to ensuring high-quality, compassionate care that respects patient dignity and supports families during a vulnerable time. This position is only part time until our census in Santa Fe grows to be able to offer Full-Time hours.
Responsibilities
Administer medications and treatments per Hospice policies and clinical standardsPerform baseline nursing skills with clinical competency and in accordance with state practice actsMonitor patient condition and promptly report any changes, needs, or concerns to the supervising RNAssist in developing and updating patient-centered Plans of Care, including measurable goals and interventionsMaintain accurate and timely documentation of nursing interventions, observations, and education providedProvide emotional support to patients and families, always upholding patient rights and dignityCommunicate effectively with physicians, interdisciplinary team members, patients, and familiesParticipate in team conferences and contribute to collaborative care planningPromote teamwork by supporting and assisting fellow staff members when neededWhat You Bring
Graduate of an accredited Licensed Vocational / Practical Nursing ProgramCurrent and valid LPN license in the State of New MexicoCPR certification and a valid drivers license with auto insuranceAt least 1 year of nursing experience (Hospice experience preferred)Strong organizational, time management, and communication skillsA cooperative, compassionate approach to patient careAbility to handle emotional demands with grace and professionalismBasic math skills for medication and treatment administrationAdditional Requirements
Maintains patient confidentiality and adheres to HIPAA and privacy policiesParticipates in continuing education and in-service trainingsKeeps current with health care regulations and standardsMay be required to participate in occasional on-call rotationWhy Work With Us?
