We are looking to fill a Remote long-term Contract role as Epic certified Healthy Planet Analyst

Required Qualifications :

• 2 years of experience and certification in Epic Healthy Planet module.
• 2 years of experience in configuring, build, and maintain the Epic Health Planet module to support population health initiatives.
• 2 years of experience in collaborating with clinical and operational leaders to design workflows that support care management, preventive care, and chronic disease monitoring.
• 2 years of experience in analysing population health data and metrics to support the design and implementation of targeted care strategies
