Description : Purpose :
The purpose of the Supervisor is to provide supervision of case managers, outreach workers and peers with lived experience on the homeless outreach team. This role also performs admission and diagnostic assessments for people experiencing homelessness who want to access mental health and substance use services. The Homeless Outreach Team provides services to adults with severe and persistent mental illness (SPMI) who are provided outreach, engagement and intensive case management services with a specific focus on relieving street homelessness.
Accountabilities :
- Provides supervision of the Homeless Outreach Team, and community-based homeless outreach workers.
- Oversees the provision of case management served by the team
- Assist in outreach of people experiencing homelessness and respond to community stakeholders and community members requests for assistance to those experiencing a housing crisis
- Assures timely documentation of services conducted by team
- Assures standards for service delivery and quality are met
- Coordinates with internal providers and community agencies and assures appropriate follow through of services for consumers
- Conducts weekly group and individual supervision as needed
- Provide education to staff on assisting consumers in obtaining safe and affordable housing
- Develop and maintain positive landlord relationships
- Assist in connecting consumers to available housing resource in the community
- Models strengths and recovery based services in the field with case managers
- Participates in departmental meetings and other meetings associated with case management and Housing / Homelessness services
- Provide oversight and management of the PATH Grant and grant requirements and deadlines
- Data reporting in HMIS and PATH PDX for SAMHSA PATH Grant Outcomes
- Provide leadership in Continuum of Care committees and other committees that work to end homelessness or prevent homelessness
- Maintain positive, effective relationships with community stakeholders, government leaders, community organizations and agencies, which directly and indirectly affect the needs and resources available in the community. Participate in CoC and other community coalitions
- Understanding of Housing First model
- Act as a steward of the organization’s resources. Ensure all resources are utilized properly, legally & ethically, and all entrusted to manage or represent the affairs of KWH are properly directed and highly effective. Ensure the organization is financially sound and functions as a viable business concern at all times.
- Develop and expand positive working relationships with current referral sources and related external partners, and continue to seek out additional community services and benefits which may be of value to clients served.
Requirements : Qualifications :
Master’s degree with a social service emphasis required.Qualified Mental Health Professional requiredFamiliar with treatment approaches and needs of adults with SPMI.Able to communicate effectively both orally and in writing.Ability to establish effective working relationships with Center staff and community agencies.Ability to recognize and be sensitive to cultural and ethnic differences.Must be able to safely perform the essential job functions with or without reasonable accommodations.
