• Perks Program : Exclusive deals and offers on products, services, and experiences you need and love.\n\nJOB SUMMARY : The Pediatric Registered Nurse plans, organizes and directs home care services and is experienced in pediatric nursing from birth through late teens, with emphasis on community health education / experience.\n\nRESPONSIBILITIES : \n
• Completes comprehensive assessments of patient and family to determine home care needs at regular established timelines. Provides a complete physical assessment and history of current and previous illness(es).\n
• Regularly re-evaluates patient nursing needs.\n
• Initiates the plan of care and makes necessary revisions as patient status and needs change.\n
• Develops the plan of care with patient's physician; establishes goals based on nursing diagnosis and incorporates therapeutic, preventive and rehabilitative nursing actions. Includes the patient and family in the planning process.\n
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician\n
• Counsels the patient and family in meeting nursing and related needs.\n
• Provides health care instructions to the patient / family as appropriate per assessment and plan.\n
• Identifies discharge planning needs as part of the plan of care development and implements prior to discharge of the patient\n
• Knowledge of high-tech nursing skills as applicable which is not limited to indwelling / straight catheter insertion / removal, nephrostomy care, ostomy care, plural and abdominal catheter drainage care, infusion services, g-tubes, and negative pressure wound therapy.\n
• Working knowledge of all infection control procedures and safety precautions.\n
• Knowledge of the nurse's scope of practice and ability to apply professional theory, practice and procedure showing appropriate application of the nursing process.\n
• Observe ethical and legal guidelines for safeguarding confidentiality of patients.\n
• Assess patient needs and physical status each skilled visit through health data assessment and patient interview. Re-evaluate patient needs through physical\n
• assessment, response to therapy and any updated physician's orders. Inform physician of any changes in patient condition, response to care, progress etc. Obtain and clarify orders for Plan of Treatment revisions.\n
• Prepares clinical notes and updates the primary physician when necessary and at least every sixty days. Completes And submits all documentation in accordance with policy and procedures.\n
• Communicates with the physician regarding the patient's needs and reports and changes in the patient's condition; obtains / receives physician's orders as required.\n
• Communicates with other health related persons to coordinate care plan.\n
• Participates in on-call duties as defined by the on-call policy.\n
• Ensures that arrangements for equipment and other necessary supplies and services are available.\n
• Instructs, supervises and evaluates home health aide / personal care aide care provided every 14 days or as stipulated by law and regulation.\n
• Instructs, supervises and evaluated LPN / LVN care provided at least every 30-days.\n
• Exercises professional management. And coordinates care between and among involved parties in home, community providers and pediatric tertiary specialists.\n\nQUALIFICATIONS : \n
• Bachelor's degree of Nursing from an accredited college / university\n
• Current active and unrestricted nursing license\n
• Pediatric care experience in the acute and / or home care environment\n
• Valid driver's license\n
• Current certification in Basic Life Support for Health Care Professionals.\n
• Minimum of 1 year of recent acute or home care experience, preferably in an institutional setting.\n
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills per competency checklist.\n
