The Connections Care Coordinator is responsible for maintaining and monitoring the memory care (Connections) holistic wellness program, including but not restricted to : assisting the Director of Engagement with input into resident assessments, leading neuroplasticity programming (Connect-Move-Learn cycles),, muscular strength, aerobic and flexibility training, wellness promotion, social-recreational activities and outings and volunteer program development and community resource coordination.
