About the Role

We are seeking an experienced and strategic Director of Pharmacy to lead all pharmacy operations for an innovative, home-based acute and post-acute care program. This role is responsible for ensuring safe, effective, and compliant medication management across home care settings, bridging clinical, operational, and logistical functions to support high-quality patient outcomes.

Key Responsibilities

• Oversee clinical pharmacy activities, including IV infusions, anticoagulation, pain management, and post-surgical meds.
• Manage medication logistics : procurement, storage, delivery, and inventory for home care programs.
• Ensure compliance with state / federal pharmacy regulations and accreditation standards.
• Integrate pharmacy systems with EMRs and remote monitoring platforms.
• Collaborate with medical, nursing, and operations teams to optimize patient outcomes.
• Lead, mentor, and grow a team of pharmacists and support staff.
• Track performance metrics : adherence, adverse events, and readmissions.
