Staff Pharmacist

We are seeking a dedicated and knowledgeable Staff Pharmacist to join our team. The ideal candidate will possess a strong background in pharmacy practice, with experience in various settings including acute care and long-term care (LTC) environments. As a Staff Pharmacist, you will play a crucial role in ensuring optimal patient care through medication management, patient assessments, and collaboration with healthcare professionals.

• Responsibilities
• Provide comprehensive pharmaceutical care to patients, ensuring safe and effective medication use.
• Conduct patient assessments to evaluate medication therapy and health outcomes.
• Collaborate with healthcare teams to develop and implement individualized medication plans.
• Manage inventory effectively, ensuring the availability of medications and supplies.
• Compound medications as required, adhering to aseptic techniques and safety protocols.
• Administer immunizations and provide patient education on medication use and health management.
• Utilize pharmacy software, EMR systems, and EHR systems for accurate documentation and patient management.
• Maintain compliance with HIPAA regulations and ensure confidentiality of patient information.
• Participate in pain management strategies and provide critical care support as needed.
• Stay current with medical terminology, anatomy knowledge, physiology knowledge, and advancements in pharmacy practice.
• Skills
• Experience in LTC pharmacy or hospital settings is preferred.
• Critical care experience is highly desirable.
• Completion of a pharmacy residency program is a plus.
• Strong clinical pharmacy experience with a focus on patient service and care.
• Proficient in medication administration techniques including IV insertion and IV infusion practices.
• Knowledge of compounding medications following established guidelines.
• Familiarity with medical management protocols for various patient populations including pediatrics.
• Ability to work collaboratively within multidisciplinary teams to enhance patient outcomes.
• Strong organizational skills for effective inventory management and workflow optimization.

Cert : BLS

License- Mass

• Ability to mix compounds REQUIRED
• Doctoral of Pharmacy (Pharm D) required
• Completion of ASHP accredited PGY1 Pharmacy Residency in Oncology is STRONGLY preferred.
• Board Certification Oncology Pharmacist (BCOP) Certification is required is STRONGLY preferred
• Must demonstrate the ability to work independently, without direct supervision within established policy.
• Applicant must be capable of supervising the work of support personnel.
• Specialty Pharmacy medication management
• Traveler must have their own vehicle for this need
