We're looking for a Patient Care Coordinator (PCC) to join our Sentry Medical team and help ensure seniors in our partner communities receive the highest level of support.

As a PCC, you'll play a vital role in connecting patients, families, facilities, and providers-making sure care runs smoothly and patients feel valued and supported.

What You'll Do

• Coordinate and manage patient care services in senior living communities
• Serve as the primary point of contact for patients, families, and facility staff
• Support providers with communication, scheduling, documentation, and follow-up
• Ensure compliance requirements are met and care is seamless
• Advocate for patient needs while building strong, trusted relationships
• Live out Ascend's mission : Serve, Lift, Unite
