The Crossings Program Coordinator is responsible for overseeing and coordinating all services for residents and families within the Crossings neighborhood. This position promotes the mission and philosophy of assisted living by leading, supporting, and empowering staff to deliver person-centered care and meaningful engagement. The Crossings Program Coordinator develops and implements recreational, social, and therapeutic programs that enhance residents' quality of life, ensures proper documentation and communication, and supports staff supervision and family relations.

Responsibilities :
Plan, implement, and promote diverse recreational programs, including music, arts, fitness,education, and community outings.
Provide at least 10 hours of weekly activities, including evenings and weekends, with optionsfor individual and group participation.
Adapt programs to meet residents' physical, cognitive, and emotional needs, ensuringopportunities for both active and passive involvement.
Prepare and post monthly activity calendars and facilitate daily engagement opportunities.Coordinate and drive residents to outings and special events as needed.Support and supervise Resident Associates to ensure quality care, hygiene, and safety withinthe Crossings Unit.
Monitor resident status, communicate changes to the Director of Health and Wellness, andparticipate in care conferences.
Conduct monthly wellness checks and ensure accurate resident documentation, assignments,and supply management.
Assist with meals, personal care, and light housekeeping as needed, maintaining a therapeuticenvironment and resident confidentiality.
Lead and support Crossings staff through regular meetings, training, and performanceoversight.
Participate in hiring, onboarding, scheduling, and evaluating staff performance.Serve in the Manager-on-Duty rotation and provide coverage as needed.Educate staff on mental health topics and assist in conducting in-service training.Build positive relationships with residents' families through consistent communication,newsletters, and family meetings.
Facilitate support groups and assist with family inquiries and concerns.Collaborate with the Executive Director and Director of Health and Wellness to promote opencommunication and family engagement.
Participate in quality improvement initiatives, safety programs, and management meetings.Maintain confidentiality and promote effective communication across departments and withoutside partners.
Perform additional duties as assigned to ensure the well-being of residents and success of theCrossings program.
Qualifications :
High School Diploma or equivalent required.Associate or Bachelor's degree in Health Sciences, Social Services, Geriatrics, Communications or a field related to the position is preferred.Experience with Microsoft Word and Excel preferred.Strong written and verbal communication skills, strong interpersonal skills, and positive community relations skills. Excellent organizational skills, detail oriented. Pleasant, professional, and personable.Ability to evaluate workers' performances, strong working knowledge of employment laws and company regulations.Self-motivated and can work independently and make decisions.Able to work well under pressure, resolve conflicts, prioritize tasks, and follow through with ideas.Complimentary professional experience in recreation or occupational therapy, art or music therapy, hospitality management, teaching, coaching, health & wellness or related fields are welcome.Current CPR CertificationApply today to learn why Distinctive Living is a certified Great Place to Work !
