Job Title : Advanced Practice Provider (APP) / Physician Assistant or Nurse Practitioner
Location : Summit County, OH (multiple clinic sites within 15 minutes of each other)
- Salary / Pay Range : $110,000 – $120,000 annually / $52.88 – $57.69 / hr;
Eligible for quarterly incentive bonuses (3–5% of salary) tied to productivity.

The Advanced Practice Provider (Nurse Practitioner or Physician Assistant) will provide high-quality, holistic primary medical and behavioral health care to patients of all ages. Working under a collaborative agreement with a physician as mandated by Ohio law, the APP is responsible for the assessment, diagnosis, treatment, and coordination of care across the lifespan.
Key Responsibilities :
Examine, diagnose, and treat acute and chronic conditions in patients of all ages.Perform wellness exams, including well-child, well-woman, and adult preventive care.Order, interpret, and act on laboratory and diagnostic imaging results.Develop and implement individualized treatment and care plans.Prescribe medications and therapeutic interventions within scope of practice.Deliver patient education focused on disease prevention, health maintenance, and care plan adherence.Provide age-appropriate and culturally competent care.Collaborate with interdisciplinary teams and specialists to ensure coordinated care.Refer patients to specialty services as needed and manage follow-up care.Participate in call rotation and provide weekend or extended coverage as assigned.Maintain accurate and timely documentation in compliance with regulations and standards.May support training or precepting of medical / nursing students.Additional Duties :
May be assigned responsibilities at multiple clinic locations within scope of practice.Involvement in supervision or administrative duties may be required.Requirements :
Minimum Qualifications :
Graduation from an accredited Nurse Practitioner (MSN-NP) or Physician Assistant (ARC-PA) program.Board certification (ANCC, NCCPA, or equivalent) with a focus in family medicine or primary care.Active, unrestricted license in the state of Ohio.Current DEA registration.
