Overview

Serve patients at home across Collin, Dallas, Denton, Tarrant, Parker, Wise and surrounding counties with a CHAP- and Medicare-certified agency committed to outcomes and patient experience.

Responsibilities

Lead OASIS SOC and ROC visits with accurate, timely documentation

Complete recertifications and safe discharges with thorough education

Communicate plan changes to providers and office staff

Ensure compliance with Medicare and agency standards using WellSky Kinnser

Champion safety, infection control, and patient advocacy in the home

Qualifications

Required

Unencumbered Texas RN license

At least 1 year recent home health experience including OASIS

WellSky Kinnser proficiency

Reliable vehicle and current driver’s license

Preferred

BSN or higher

Prior hospice, wound care, or chronic disease education experience
