Key Responsibilities
- Provides orientation to new patients to the programs' rules, policies, procedures, and patients' rights.
- Conducts Intakes, Discharge Planning, Initial Assessments, and Initial Treatment Plans within time guidelines as required by applicable federal and state regulation, CARF guidelines and Pinnacle Policies.
- Involves patients in assessment, treatment planning, and intervention to support their recovery.
- Administers the biopsychosocial assessment at intake and on an ongoing basis.
- Utilizes SNAP (Strengths, Needs, Ability, and Preferences) to develop individualized Treatment Plans aligned with patients' goals.
- Determines the clinical necessity of counseling services based on assessment findings.
- Monitors and documents changes in patients and relapse indicators to support relapse prevention interventions.
- Complies with Medical QA and Clinical Risk Management Policies, including evaluation of high-risk conditions and use of screening tools (BAM-R, C-SSRS, GAD-7, PHQ-9).
- Reviews and coordinates with other practitioners and refers patients as needed, maintaining appropriate releases of information.
- Provides professional counseling and referrals, addressing clinical and special needs and coordinating with community resources and referral partners.
- Delivers individual and group counseling, applies methods such as Cognitive Behavioral Therapy and Motivational Interviewing, and schedules regular sessions.
- Documents counseling entries per federal and state regulations, CARF guidelines, and Pinnacle policies.
- Participates in Community Relations and Outreach; attends required training and case conferences.
- Attends team meetings and completes training courses timely; collects research data when requested; may observe urine sample collection as required.
- Performs other duties as assigned.
Qualifications
Counselor I
$20 / hour to $21 / hour
Possess an associate degree in a relevant field and required credentials to provide counseling services.Kern County may require a bachelor's degree or high school diploma / GED with 75% certification education.Counselor II
$21 / hour to $24 / hour
Possess an associate degree or equivalent certification; 2-5 years of experience or Master level preferred; require counselor registration / certification by a recognized organization; 75% of required coursework may be acceptable in Kern County.Counselor III
$24 / hour to $30 / hour (varies by certification and licensure)
Must have licensure or certification as an Alcohol and Other Drug Counselor (CA approved), or licensed / license-eligible with the appropriate board; immediate credentialing required; ongoing supervision as needed.Benefits and Career Path
