Job Summary (in List Format) : Licensed Vocational Nurse (LVN) Skilled Nursing Facility (SNF)
- Uniform Requirement : All nursing staff must wear Caribbean Blue scrubs.
- Reports To : Director of Nursing (DON).
- Collaboration : Works closely with physicians, nursing staff, residents, families, and other facility departments to provide individualized nursing care.
- Patient Care : Conducts assessments, develops care plans, performs treatments, administers medications, and provides direct and individualized patient care.
- Documentation : Maintains accurate patient records, documents assessments, treatments, and patient progress.
- Emergency Response : Responds appropriately to emergencies, initiates resuscitation measures, and follows protocols.
- Education & Training : Provides patient / family education, assists with orientation of new staff, and participates in ongoing education and staff meetings.
- Safety & Infection Control : Adheres to safety protocols, infection control standards, and ensures safe use of equipment.
- Professionalism : Maintains confidentiality, demonstrates professional behavior, and upholds hospital mission and ethics.
- Teamwork : Assists and collaborates with other departments and staff; acts as a team player and supports CNAs as needed.
- Communication : Communicates clearly and respectfully with patients, families, and staff; reports patient conditions as required.
- Regulatory Compliance : Maintains required licensure and certifications (CA LVN license, BLS / CPR), and complies with all regulatory and facility requirements.
- Work Schedule : Night shift; flexibility required to work beyond normal hours or in other positions temporarily as needed.
- Physical Requirements : Must have stamina, physical and mental health to perform patient care duties (walking, bending, lifting, etc.).
- Other Duties : Performs additional duties as assigned by nursing management in accordance with facility needs.
