Job Summary

The Director of Nursing is accountable for the overall direction, coordination, and evaluation of care and services provided to Touching Hearts clients.

Responsibilities

Tasks and responsibilities correlated with this position include (but are not limited to) the following :

• Lead and guide clinical activities, including but not limited to patient assessments, management of care plans, home evaluations, medication management, and other patient services
• Ensure that federal, state, and local laws, as well as professional standards and principles in the nursing profession, are respected and comprehended; communicate and reinforce company policies and procedures
• Provide direct supervision and support for Certified Nursing Assistants (CNA), Home Health Aides (HHA), and Companion Caregivers
• Collaborate with clients, families, caregivers, and other care professionals surrounding the client, to ensure the highest quality of care is given; conduct client assessments to assess client needs, communicate needs to the family members and Touching Hearts care staff
• Co-conduct clinical training for Touching Hearts care staff, as mandated for compliance; assist with the development of continued education for caregiver teams
• Assist with clinical and caregiver annual performance evaluations
• Maintain accurate departmental and staffing records for reporting
• Manage clinical on-call responsibilities to support Touching Hearts care staff and clients; maintain clinical teams on-call schedule
• Operational tasks to further the success and support of Touching Hearts caregiving teams and clients

This position reports to the Executive Director of Operations.

Skills and Qualifications

Salary & Perks

$70,000 - $80,000 annually This is a full-time salary, exempt position. There will be an evaluation of 90 days, to assess the mutual fit, accuracy of the job scope, and assure you have the support you need to continue your success. During the 90 days evaluation, a salary increase may be offered.

Working Environment

Touching Hearts at Home, Metro offices are located in Plymouth and Bloomington, MN. Both offices are collaborative, open office environments. In support of our Caregivers, this role requires the ability to commute to / from client locations, anywhere within our territory to provide care for our clients.
