Position Purpose : Deliver mental health services & ancillary support services to clients who reside in Permanent Supportive Housing unit resources. Documentation and completion of reports to meet the requirements of Grant deliverables. Provide program services in an ethical, legal and moral manner within a safe and therapeutic environment, consistent with applicable regulatory and accreditation standards.

Job Requirements :

• High School Diploma required, Bachelor's Degree preferred.
• Minimum of one-year experience in mental health setting.
• Valid Driver’s License with clear driving record and valid auto insurance.
• Self-starter with ability to work independently
• Ability to work within a team
• Belief in the mission and vision of Rosecrance

Work Location : Rosecrance Ware Center – Rockford, IL
