iCare Health Network is looking for a Regional Admissions Coordinator.
Essential responsibilities :
Assessing and determining the appropriateness of admissions to the iCare managed facilities and facilitates client in-takes and calls
Obtain required information and admit residents in an efficient manner, in accordance with established iCare policies and procedures.
Strong knowledge of local healthcare market. Ability to travel regularly within your community to multiple iCare locations.
Work with the hospital based clinical liaison team to evaluate residents for medical and psychosocial appropriateness.
Determine and input ICD9 coding based on resident’s diagnosis.
Facilitate the PASRR review process.
Interacts with residents and families to coordinate services.
Ensure that all required admission paperwork is obtained.
Determine appropriate location for admission based on diagnosis and other clinical factors.
Maintain facility bed availability.
Follow up with new residents prior to their arrival date at facility. Discuss clinical and other concerns with clients and facilities.
Coordinate health insurance benefits for residents, including Medicaid.
Understand current facility programs and offerings, and communicate that knowledge to potential clients.
Maintain facility waiting lists.
Be able to handle multiple cases on a daily basis in a calm and professional manner.
Secure patient referrals from key healthcare organizations and submit reports on rate of conversion to admission.
Pre-admission on-site visit with medical record review, patient / responsible party and hospital caregiver interviews to facilitate coordinated care upon admission to skilled facility.
Ability to obtain / assess quickly with new referral
Assess and communicate patient healthcare needs prior to admission.
Coordinate admission with the team.
Extensive communication with physicians and mid-level providers from multiple referral sources such as tertiary care centers, physicians’ offices, cardiology services and many more.
Extensive rounding within one or more care centers and manages a daily caseload of specific patients.
Daily review of planned admissions and readmissions as well as readmissions to hospital within 30 days of admission to skilled facility.
Deploys new initiatives in clinical interventions, new clinical guidelines and programs and controls related to readmissions and length of Coordinates the work of employees to meet patient goals and expectations.
Participate on project teams with acute and post-acute providers to develop processes, tools and programs to reduce avoidable readmissions and improve patient outcomes.
Assist patient and family as a resource in education as they transition through the health care continuum.
Assists with the resolution to all short stay patient / family concerns. Communicates opportunities for improvement to Department Managers.
Assist with planning and managing positive discharges and outcomes including transition to home care services.
Facilitates re-integration into community with social services staff, patient, responsible party and post skilled facility partners to include primary care physician and Touchpoints at Home or other homecare agency.
Position is based out of corporate office with remote opportunities.
Qualifications :
High school diploma or GED. A degree in social work, healthcare management or other applicable field of study is preferred.
Must have at least two (2) years of experience in an admissions, social services, or intake role in a human services or geriatric setting. Previous skilled nursing admissions experience in a fast-paced setting is preferred.
Hospital and / or Long Term Care experience of five years or more is preferred. Relationship building, corporate, development or previous care transitions experience is highly desirable and an important element in securing each patient as they have many options to choose from. Ability to multi-task and balance a multitude of competing priorities a must.
Must be proficient with various software programs such as WORD, EXCEL, and PUBLISHER and be capable and able to achieve proficienty with numerous web based platforms and electronic medical records used in the admissions process.
