• Assist the men with scheduling and coordination of health care specialists\n
• Accompany Jesuits to physician or specialist appointments as needed\n
• Report changes in condition and relevant health updates to the nurse on duty, Care Coordinator Nurse, and Jesuit leadership\n
• Manage admissions and readmission, including communication of condition, treatments, and medications\n
• Ensure care plans are accurately and promptly communicated to the Care Coordinator Nurse and / or floor nurse on duty\n
• Assume full responsibility for assigned cases\n

• Current Registered Nurse (RN) license in the State of Missouri\n
• CPR certification required\n
• Minimum of one (1) year experience in an adult medical / surgical unit\n
• Six (6) months of home care experience preferred
