Position Summary :
The Patient Care Supervisor is responsible for providing administrative, planning and coordination assistance to the DPCS.
The position will supervise HHA, LVN and other admin staff in the office. Collaborate with agency departments, facilities and physicians as needed to coordinate appointments and or obtain docs required for care and billing.
This position must be Licensed and must have the ability to receive a physician order. This position will manage daily / weekly / monthly reports, patient supplies and interagency / community referrals.
ESSENTIAL JOB FUNCTIONS / RESPONSIBILITIES
- Oversee Scheduling Department, LVNs / HHAs daily tasks
- Participate in the interview process for new LVN, HHA and admin personnel, and has the ability to hire and fire
- Supervise scheduling department to verify employee / patient schedules are accurate, staff are productive & schedule variances are reconciled.
- Approve and monitor missed visits and direct scheduling to make certain visits are rescheduled / staffed appropriately.
- Responsible for running daily / weekly / monthly reports.
- Support patient needs by communicating with field staff when patient assignments need priority i.e. clinical change in condition, with direction from DPCS.
- Supports safe, effective care delivery by assisting with order review to ensure State and Federal Regulations are satisfied.
- Responsible for running daily / weekly / monthly reports.
- Assist maintaining patient satisfaction and improving satisfaction ratings.
- Other duties as assigned may cover field staff in situations of high census etc.
- Ensure personnel development including orientation for new staff (LVN, HHA, scheduling)
- Facilitates patient care coordination documents with acute and post-acute facilities during patient transfers.
- Assists with care coordination i.e. DME, medical supplies, coordination of appointments or referrals to outside agencies.
- Answer incoming calls, triage calls for medical issues, and documenting complaints.
- Investigate and resolve patient complaints complaint must be escalated to the DPCS / Branch Director if the issue is not able to be resolved.
- Instruct and guide clinical staff (LVN and HHA) to promote more effective performance and delivery of quality of care
- Lead case conference meetings with organization personnel to facilitate coordination of care
- Direct Emergency / Disaster Triage
POSITION QUALIFICATIONS
1.Must be a Licensed LVN with a valid and current license to practice in the state of California.
2.At least two to four years of experience in home health
3.Knowledge of business management, governmental regulations, and standards.
4.Must be a licensed driver with an automobile that is insured in accordance with state and / or organization requirements and is in good working order.
SKILLS REQUIRED
1. Skilled in making effective verbal and written communications with subordinates, co-workers, consumers, referral agencies and comfortable with conflict resolution skills.
2. Ability to educate consumers on all service lines and present to seniors and aides in a group setting
3. High energy level and passionate about care delivery
4. Must have excellent organizational skills and ability to complete competing priorities
5. Ability to listen attentively and offer care options based on individual patient health needs
