Education Qualifications : Full time position involving Heart Failure services.Clinic is Mon-Fri 8a-5pTreat clinic patients (10-13 per clinic day)Heart Failure clinic and follow-upsOutreach / telemedicine 2-3x per month in regional locationsWork collaboratively on a team-based model that includes a mix of Cardiologists and Advanced Practice Clinicians and a strong team of nurses, techs and ancillary staff within our Heart & Vascular Department.
Within the CHF clinic, you will partner with colleagues in managing the Heart Failure population across the continuum, resulting in some of the best outcomes in the nation.
