The WISE Prevention Counselor is directly responsible for the delivery of WISE / SBIRT services to the Older Adult and Veterans population.
This position reports directly to the Senior Prevention Counselor.
Essential Duties and Responsibilities :
Core duties and responsibilities include the following. Other duties may be assigned.
Participate in required Wellness Initiative for Senior Education Training (WISE) evidence-based program training
Co-facilitate WISE EBP with groups of Older Adults and / or Veterans, to fidelity, in-person or virtually
Participate and complete Screening and Brief Intervention for Referral to Treatment ( SBIRT) 12-hour training .
Conducts assessments for identification of WISE participants that are would be benefit from SBIRT intervention.
Facilitate individual SBIRT rap sessions.
Conducts community drug education forums.
Assist Older Adult Care programs and Veterans Services Programs for WISE / SBIRT implementation
Collaborate with certified PASAS treatment providers to facilitate warm handoff referrals, as indicated from SBIRT tool
Assist Senior Prevention Counselor, create and convene Advisory Committee consisting of the following stakeholders : OAC, MAT and other organization
Liaison between referral sources.
Participates in team meetings for the development of work plans.
Participates in staff meetings.
Attends on going trainings for skill development.
Oversight of meal accompaniment as part of WISE implementation, regardless of mode
Data collection and aggregation
Provide information and data preparation of required reports.
May be required to work evenings and weekends.
Education
Bachelor’s degree in human services / social service and / or
CASAC (Credentialed Alcoholism and Substance Abuse Counselor)
Training is SBIRT (preferred)
Experience :
Desire and / or experience working with the Older Adults and Veteran’s population
3 years (bachelor’s degree) or 4 years (CASAC )experience working in a non profit setting
Experience in group facilitation of evidence-based programming or practices
Must have computer skills.
Must successfully complete a criminal background check, obtain clearance from the Office of Children and Family Services and be drug free.
