Position Summary :
We are seeking a dedicated and passionate Inclusion Infusion Pharmacist to join our team in Pine Brook, NJ. The Inclusion Infusion Pharmacist will play a pivotal role in ensuring that our infusion services are delivered with a focus on inclusivity, accessibility, and cultural petency.
This position offers a unique opportunity to contribute to the enhancement of healthcare services for diverse patient populations.
Duties and Responsibilities :
- Collaborate with healthcare providers to develop culturally petent infusion therapy plans tailored to individual patient needs.
- Provide pharmaceutical care and medication management for patients receiving infusion therapy.
- Conduct patient assessments to identify barriers to care and develop strategies to address them.
- Advocate for the needs of underrepresented munities and ensure that healthcare services are accessible to all patients.
- Participate in munity outreach initiatives to promote health education and awareness.
- Stay updated on best practices, guidelines, and regulations related to infusion therapy and cultural petency in healthcare.
- Maintain accurate patient records and documentation in pliance with regulatory standards.
Requirements :
- Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
- Active pharmacist license in the state of New Jersey.
- Minimum of 1years of experience in infusion pharmacy practice.
- Strong understanding of cultural petency and its importance in healthcare delivery.
- Excellent munication and interpersonal skills.
- Ability to work collaboratively in a multidisciplinary team environment.
