Your day as a PCHH RN Case Manager-Sedalia

As a new PCHH RN Case Manager at Katy Trail Community Health in Sedalia, you can expect to provide individualized patient care plans, coordinate services, and collaborate with a multidisciplinary team to ensure the best outcomes.

What you need to be successful

To excel as a PCHH RN Case Manager at Katy Trail Community Health in Sedalia, you must maintain a valid RN licensure in Missouri, showcasing your commitment to professional standards.

With at least three years of experience in a clinic setting, you will bring valuable expertise to the role. While experience in a community health center is preferred, your ability to adapt and learn quickly is essential.
