Job Description
The Family Support Specialist will provide person-centered, habilitative services to each participant served. These services are designed to assist individuals in gaining independence and exercising personal choice while building on their life skills and enrichment opportunities.
FSs provide support for participants with disabilities through training and proximity supervision. Services are designed through choice, observation, and assessments to ensure the effective delivery of services.
FSS communicates observations and needs related to participant-identified goals to the Program Director. FSs may support participants in their residence or community based on program and choice.
FSs should avoid situations where their interests conflict with their job functions and immediately disclose the possibility of a conflict of interest to their supervisor.

Job Duties (not limited to) :
- Support the Individual Service Plan (ISP) by engaging in the team’s decision while providing input and collateral from services provided.
- Tracks progress towards goals and objectives in ISP.
- Facilitates opportunities for people to gain independence while building on choice life skills and enrichment activities.
- Assist in the development and implementation of choice programming.
- Increases or Decreases support as dictated by the progress of the ISP goals and objectives.
- Complete written documentation daily or as written in the program identified.
- Maintain accurate records on support services provided.
- Document and recommend actions to increase the independence of people supported, as needed.
- Maintain all applicable certifications, including First Aid and CPR.
- Responds to crisis by policy and training.
- Transport clients when necessary, using personnel or Angel Guardians, Inc. Vehicles.
- Makes decisions based on Agency Training and Mission.
- Flexible hours may include evenings and weekends.
- Other Duties as assigned.

- Ability to foster relationships in the community or life of the participant served.
- Ability to have objective written and verbal communication / documentation skills.
- Ability to engage in a team setting, supporting the participant's best interests.
- Is a positive role model for people supported.
- Ability to adapt to changes in the environment or assist in behavior modification.
- Valid driver's license and good driving record.
- Clearance of all applicable background checks.
- Ability to read and interpret documents, write specific reports and correspondence, apply mathematical concepts to practical situations, solve complex problems, and interpret written and verbal instructions is required.
Physical Requirements :
- Personal Assistance : Ability to assist individuals in activities of daily living and related job tasks of programming.
- Attire : Dresses appropriately for the day and activity engaging in.
- Lifting / Physical Demands : Able to lift 20 or more pounds as needed. Ability to move throughout the facility and community as required.
- Challenging Behaviors : Ability / willingness to support individuals with challenging behavior.
Education and Experience :
- A high school diploma or its equivalent.
- The ability to maintain First Aid and CPR certification.
- Experience with adults with developmental disabilities (or similar experience is strongly preferred).
